Case Management is a collaborative process that assesses, plans, implements, and monitors services to help individuals and families meet their comprehensive health needs. It involves coordinating care, advocating for resources, and promoting positive outcomes through a combination of assessment, planning, facilitation, and evaluation.
For our team this is a long-term, integrative process that involves working with an expert advocate to identify what the care needs of your family member or loved one are and create a plan for addressing those needs. We work closely with individuals and their families to provide comprehensive support for their treatment, recovery, and supported living needs. Our services include:
- Clinical Records Review & Needs Assessment
- Crisis Response Coordination
- Treatment Planning & Placement
- Care Advocacy and Family Coaching
- Transitional & Supported Living planning and placement
- Ongoing Oversight & Evaluation of Care Needs
We work with our clients from 3 to 6 months with some establishing annually renewing long term care agreements. We monitor the entire process and advocate on behalf of the family and the person of concern to ensure care needs are met and your loved one is able to achieve the highest level of sustainable independence.
Our team of trained professionals specializes in mental health and substance use treatment having worked with programs and providers across the country. Through our services we provide individualized support and guidance for families and individuals challenged with severe and persistent mental health and co-occurring substance use conditions. Conditions including complex trauma, Autism, personality, mood, and thought disorders are among the broad range of diagnostic profiles we work with.

Planning the critical steps needed for effective treatment and long-term recovery can be difficult. We offer semi-annual and yearly case management agreements to develop long-term plans, optimizing the recovery and lifestyle of your family member. In this area, we may also confer with the family’s attorney, financial planner, care manager, or trustee. Identifying and coordinating treatment and long-term care resources is a difficult job if you don’t know your way around.
